Novo Nordisk's GLP-1 Growth Engine: Why Strong Demand Is Not Producing Strong Revenue Growth

 

Novo Nordisk is a Danish pharmaceutical company that has worked on diabetes treatment for more than a century. The reason it came to be valued as a very different kind of company during the 2020s is that one class of medicine, the GLP-1 receptor agonists, expanded from diabetes into obesity and then into cardiovascular risk reduction.

Yet the second-quarter 2026 results show a gap: the products are selling well, while growth for the company as a whole is low. This article examines where that gap comes from.

Rather than listing figures, it follows the structure through which revenue growth is actually produced.

1. Why the GLP-1 Business Was Built This Way

Semaglutide is a single active substance, but it is sold as different products depending on the indication and the formulation. Ozempic for type 2 diabetes, Rybelsus as an oral diabetes treatment and Wegovy for obesity all originate from the same molecular family.

The reason for this structure is straightforward. Extending indications from one substance carries a lighter clinical and manufacturing burden than developing an entirely new molecule from the beginning.

The decisive moment in that expansion was 8 March 2024. The US Food and Drug Administration (FDA, the American medicines regulator) approved an expanded indication for Wegovy to reduce the risk of major adverse cardiovascular events in adults with established cardiovascular disease who also have obesity or are overweight. In the SELECT trial that supported the decision, the primary composite endpoint occurred in 6.5% of patients in the treatment group and 8.0% in the placebo group.

With that approval, Wegovy shifted in character from a weight-loss product to a medicine for long-term management of a chronic condition. Once long-term use is assumed, revenue becomes recurring rather than one-off.

2. The Structure and Requirements of the Wegovy Range

As of 2026 the Wegovy range has three branches, each approved under different conditions.

Data as of 12 August 2026. The condition worth noting is the one attached to the higher dose. Wegovy HD is approved for people who have tolerated the 2.4 mg dose for at least four weeks and for whom additional weight reduction is clinically indicated. It is not a starting dose.

The oral formulation plays a different role. It serves as an entry route for people who are reluctant to use injections. Novo Nordisk stated that, on its own internal data, US prescriptions of the Wegovy pill exceeded 265,000 in the week ending 17 July and that cumulative prescriptions since launch had passed five million. The company published its own figures on the grounds that third-party prescription trackers capture only around 60% of a market that has shifted towards telehealth and self-pay distribution.

3. What the Second-Quarter 2026 Data Shows

Sales in the second quarter of 2026 were DKK 78,488 million. Reported sales grew 3% at constant exchange rates (CER), while adjusted sales, which exclude one-off items, grew 7% at CER and adjusted operating profit grew 11%. Reported operating profit reflects non-cash impairment charges of DKK 6.3 billion related to intangible pipeline assets.

The bars in the chart are not calculated on the same basis. The first two are adjusted figures at constant exchange rates; the remaining three are as reported in US dollars. They should be read with that difference in mind.

The longer-term trend in the business itself remains steep. According to the 2025 annual report, obesity care sales grew from about DKK 6 billion in 2019 to DKK 82,347 million in 2025. For 2025 alone, obesity care sales increased 26% in Danish kroner and 31% at constant exchange rates.

4. The Core Mechanism Behind Revenue Growth

This is where product demand and company growth come apart. GLP-1 revenue is produced through roughly four stages.

First, prescription volume increases. Second, it is determined which formulation and which distribution channel those prescriptions flow through. Third, realised prices differ by channel. Fourth, what remains is recognised as revenue.

Novo Nordisk stated that in the second quarter GLP-1 volumes grew while the company was affected by lower realised prices. In other words, it has entered a period in which volume growth does not translate directly into revenue growth.

The oral formulation adds a further complication. If the tablet simply moves demand across from the injectable products, total company revenue does not increase greatly. If instead it brings in new patients who were not previously using injections, the market itself expands. That is why prescription counts for the oral formulation alone are not sufficient grounds to conclude that revenue will rise.

5. How Eli Lilly's Structure Differs

Both companies operate in the GLP-1 market, but their structures are not the same.

Data as of the 12 August 2026 US market close. The price-to-earnings ratio is the closing price divided by trailing twelve-month earnings per share.

Two differences matter most. The active substances belong to different classes, and competition in oral formulations has already begun. The Medicines and Healthcare products Regulatory Agency (MHRA, the UK medicines regulator) approved Foundayo on 10 August 2026, its first approval outside the United States.

6. Limitations of the Current Business Structure

The first limitation is price. Even when volumes rise, falling realised prices reduce the increase in revenue. Eli Lilly also reported that in the same quarter volumes grew 60% while realised prices fell 13%. Pricing pressure is not confined to one company.

The second limitation is the guidance itself. On 5 August Novo Nordisk raised its 2026 full-year outlook for both adjusted sales growth and adjusted operating profit growth to 0% to -6% at constant exchange rates. Even after the upgrade, the top of the range is 0%, which means a recovery in growth has not yet been confirmed in the numbers.

The third limitation is next-generation competition. Amgen is running MARITIME, the phase 3 programme for MariTide, which targets dosing once a month or less frequently; its phase 2 study reported mean weight loss of up to about 20% at 52 weeks. Viking Therapeutics has completed enrolment in the phase 3 trials of subcutaneous VK2735 and expects to begin phase 3 testing of the oral formulation in the fourth quarter of 2026.

The industry is not contracting. The World Health Organization states that about 16% of adults worldwide were living with obesity in 2022 and that adult obesity prevalence has more than doubled since 1990. The market is growing, but market growth and the growth rate of a single company are not the same thing.

Volumes grew but realised prices fell, and the full-year outlook was raised while its upper bound remained at 0%. Whether the structure has changed can only be confirmed by reading volume, price and market share together in the next set of results.
